New Resolve mdx TRF is LIVE (R11)

Resolve mdx TRF (PL-FORM-0095-R11, attached) is  live. Therapak is distributing  the new version of the TRF with each order.

These revisions are intended :

• To reduce problem holds,

• To  improve reimbursement
• Make urine collection easier

As discussed in the training, changes to this TRF include:

1. Test Ordered
   1. “Resolve mdx UTI Testing” is Pre-checked
   2. Clients can Add STI Testing by checking the box below the UTI Testing box
   3. Clients can order “STI Testing only” by selecting the box in the Custom Testing box
   4. “Selected Testing” orders will go on hold

2. Billing Information
   1. ICD-10 Codes Updated

3. Only one tube of urine required
   1. Clients who order both UTI testing and STI testing for the same patient no longer need to submit two tubes of urine. One 3mL tube of urine will suffice
   2. Note: both Tests must be selected at the time of submission
